Home Health Care is the most valuable benefit of Medicare which reflects patient's preferences. If you are covered for benefits under Medicare A, our services may be available to you at no cost. We also accept many private payers. 

We will help you rehabilitate from medical conditions requiring supervision or assistance with your daily life. Blue Mountain Home Health Care, Inc. takes pride in helping you rehabilitate from medical events like a stroke or surgery, or managing chronic conditions such as diabetes, heart disease, and arthritis, or other conditions requiring some supervision or assistance with your daily life.

Skilled Nursing staff at BMHHC INC. will work with your physician to assess an appropriate Plan of Care for you within 48 hours or sooner after the referral to our Intake Coordinator office. Services available to you will include but are not limited to:

• A Comprehensive initial medical assessment 
• Specialized Care including hematology, oncology, chronic condition monitoring, and diabetic care that includes patient education and management for foot care, insulin managing, proper nutrition, and Psych nursing.
• Psychiatric Nursing Care for Mental Health: Alzheimer and PTSD, anxiety or panic attacks, exacerbations of chronic schizophrenia including depression. Learn More> 
• Cardiac and Pulmonary Care include patients experiencing cardiomyopathy, Congestive Heart Failure (CHF), Hypertension, pre/ post open heart surgery, Chronic Obstructive Pulmonary Disease (COPD) with cardiac involvement or post coronary angioplasty. 

          Our goal is to improve your overall health significantly at your home by controlling risk factors
          with regular monitoring, treatment, and education about cardiac and pulmonary disease. 

• Wound Care Service to provide direct patient care involving incontinence, draining wounds, and pressure ulcers. Wound Care also includes patient education and nutritional assessment and instruction for optimal wound healing. 

         Our skilled nurses provide treatment and services for pressure ulcers, traumatic ulcers, 
         neuropathic ulcers, education in prevention of complications, and infections.

• Infusion Therapy services include cardiovascular therapy, immune globulin, hydration and electrolytic, pediatric therapy, and blood factor therapy.  An RN or Physical Therapist is assigned as case manager to each patient. We also provide nutritional support to patients receiving enteral or parenteral feed.
• Pediatric Care is our specialty in complex, high risk, technology dependent children care.   We provide short- term or Long- term adult or pediatric medical home care. Learn More>
