Medical Social Worker

Medical Social  Work (MSW) service is covered by Medicare as dependent service. Blue Mountain Home Health Care, Inc. (BMHHC INC) provides medical social worker consultation to patients, as part of the admission process to assist patients to maintain or improve their medically related social, emotional, and physical well-being. 

We provide Licensed MSW Services on a short-term basis (1 to 2 visits) to a patient's family member or caregiver when it is established that a brief intervention is needed to remove a clear and direct impairment to the effective treatment of a medical condition. Services that our medical social workers provide include: 

• Complete assessment of the care needed for the emotional and social factors directly or indirectly related to the patient's medical condition, and for adjustment to care in accordance physician's orders, and State regulation.
• Facilitation of support group and helping patient to other resource in the community in resolving patient's problems.
• Provide crisis interventions on bio- psychosocial issues. Assist patients to understand follow medical recommendations. 
• Co- ordinates for patient, family, and Physician conferences to promote quality care.
